ESTABLISHING A WATER ACCOUNT

The following is required to establish a water account with SWSA:

  1. Completion of an application for service including a signed water users agreement
  2. A photo I. D.
  3. A legal document proving date of residence. Accepted documents include a settlement statement (from closing papers), a warranty deed, or a copy of property tax valuation notice. Renters must provide a copy of their lease or rental agreement. The document must show the date of residence, the property owner's (or renter's) name and correct property address. If you are a renter and your landlord does not provide you with a formal lease or rental agreement, you may use our Rental Verification Form. The rental verification form provided by SWSA must be completed and signed by the landlord before service can be established.
  4. Payment of the $75.00 account set up fee.

    We can not provide same day service for new account connection.  Please allow up to three (3) business days for processing and actual connection to the system.

CLOSING A WATER ACCOUNT

The account owner must complete a “Request for Final” form at the office.  If it is not possible to come to the office due to having already moved from the area, we can FAX this form for a fee of $1.00. You may also close your account by downloading and printing the "Request for Final".  The form must be notarized and returned to our office.  The vacating account owner must provide a forwarding address for the final and a new phone number if possible.  All fees associated with the account must be paid in full by the vacating account owner.  We do not offer same day service for disconnection.  Please allow up to three (3) business days for processing the disconnect order. 

Most of the forms required for various activities are available below.  There are, however, two (2) forms which will require the customer’s physical presence to complete.  The Water User’s Agreement and the Request For Final will require that the customer come in and, after presenting a photo I.D., sign the forms in the presence of a Smiths Water and Sewer Authority employee.
